Are you a former cadet or midshipman from the United States Military Academy, or the United States Air Force Academy, or United States Naval Academy AND you:
Completed the prescribed course of instruction and graduated from the academy not more than two years ago AND
You were determined medically disqualified from completing a period of active duty service in the armed forces (not from gross negligence or misconduct)?
If you answer yes to this question you must submit a copy of your latest DD-214 Certificate of Release or Discharge from Active Duty. Any copy that shows a medical discharge, all dates of service, as well as character of service [Honorable, General, etc.] is acceptable OR a VA letter that shows medical discharge, dates of service, AND character of service. If you have more than one DD Form 214 for multiple periods of active duty service, you should submit a copy for each period of service. If you were issued a DD-215 to amend aforementioned information on the DD-214 you must submit that too.
Failure to provide supporting documentation will result in a rating of ineligible.
