BASIC REQUIREMENTS- We will evaluate your resume and responses to this Assessment Questionnaire to determine your qualifications for Intermittent/Per Diem Nurse Practitioner (Home-Based Primary Care)-Santa Rosa position. NP LICENSURE: Do you possess a current, full, active and unrestricted licensure or otherwise recognized as a nurse practitioner in a State, the District of Columbia, the Commonwealth of Puerto Rico or a territory of the United States? (Please provide a copy of your licensure).

NP CERTIFICATE: Do you possess a full and current certification as a nurse practitioner from the American Nurses Association or another nationally recognized certifying body in case management or ambulatory care? (PLEASE PROVIDE A COPY OF YOUR CERITIFICATION FOR VERIFICATION)

DRIVER LICENSE: Do you possess a current and valid State issued driver's license?

DEA PREFERRED: Do you possess a current Drug Enforcement Administration (DEA) Registration?

PREFERRED NP EXPERIENCE LEVEL: Do you possess at least 2 to 3 full years of Home Base Primary Care or Community Home Care/Hospice experience?

Have you review the "REQUIRED DOCUMENTS" section of the announcement and supplied all required documents [e.g., resume/CV, OF-306, VA 10-2850 variations, SF-50 (if civil service federal employee), DD-214/SF-15/Preference Letter (if Veteran), etc.]?
Merit Promotion Documentation Reminder- If you are a current Federal employee, you must provide a copy of your last or most recent SF-50, "Notice of Personnel Action." An SF-50 showing your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade.
- Award 50's do not provide sufficient information and will not be accepted for the purpose of verifying time-in-grade.
- Examples of appropriate SF-50's include:
- Within grade increases at the highest grade held; or
- Promotions with an effective date more than one year old; or
- SF-50's at the highest grade held with an effective date more than one year old.
