1. BASIC REQUIREMENTS: Select the statement below that best describes your ability to meet the basic requirements for this position. Please refer to the Qualifications section of this vacancy announcement for detailed information:

I meet this qualification requirement and my transcript(s) are attached: I have a bachelor's degree or higher from an accredited college or university in medical laboratory science, medical technology, clinical laboratory science or in a related science (e.g., laboratory sciences such as chemistry, biochemistry, biology, microbiology, immunology, etc.) AND I have completed of a medical technology clinical practice program. Clinical practice programs completed after 1974 must have been accredited by the National Accrediting Agency for Clinical Laboratory Sciences (NAACLS), Commission on Accreditation of Allied Health Education Programs (CAAHEP, formerly CAHEA), or the Accrediting Bureau of Health Education Schools (ABHES). The professional curriculum may have consisted of a post-baccalaureate certificate program or be integrated into a four- year program of study that culminated in a baccalaureate degree.

B. I meet this qualification requirement and my transcript(s) are attached: I have a bachelor's degree from an accredited college/university, including 16 semester hours of biological science (with one course in microbiology), 16 semester hours of chemistry (with one course in organic or biochemistry) and one course in mathematics AND I have two years of post-certification clinical laboratory experience (e.g., blood banking, chemistry, hematology, microbiology, immunology, clinical microscopy, etc.) within the last ten years as a certified Medical Laboratory Technician by the American Society for Clinical Pathology Board of Certification (ASCP-BOC), American Society of Clinical Pathology Board of Registry (ASCP-BOR) or American Medical Technologists (AMT). This experience must include performing moderate and/or high complexity testing in blood banking, chemistry, hematology, microbiology immunology and clinical microscopy (or categorical experience which matches categorical certification) in a clinical laboratory.

C. I meet this qualification requirement and my transcript(s) are attached: I have a bachelor's degree from an accredited college/university, including 16 semester hours of biological science (with one course in microbiology), 16 semester hours of chemistry (with one course in organic or biochemistry) and one course in mathematics AND I have five years of clinical laboratory experience within the last ten years performing moderate and/or high complexity testing in blood banking, chemistry, hematology, microbiology, immunology and clinical microscopy (or categorical experience which matches categorical certification) in a clinical laboratory.

D. I meet this requirement as a Foreign Graduate and my transcript(s) are attached: I am a graduate of a foreign baccalaureate degree program with the degree found to be equivalent to degree programs recognized by the NAACLS or clinical laboratory experience as described in option b or c above. This finding may be based on either of the following: (1) A letter from a college or university with a baccalaureate program recognized by the NAACLS stating that the individuals foreign degree has been evaluated and been found to be equivalent to its Bachelor of Medical Technology degree OR (2) A letter from ASCP-BOC or AMT stating that the individual is eligible for the certification examination.

E. HPTs: I am a VHA Health Professional Trainee (HPT) who has successfully completed or is expected to successfully complete an affiliate sponsored or VA sponsored training program and on track from meeting the education requirement at the time of appointment. I understand that I must provide a proof of HPT participation to be considered under this program. (Documentation required with application; e.g. SF-50 or VA Scholarship letter.)

F. I do not meet any of the above education or education and experience requirements listed above.

CERTIFICATION: Candidates must currently possess the appropriate certification as a Medical Technologist (MT), Medical Laboratory Scientist (MLS), or Clinical Laboratory Scientist (CLS) given by the ASCP-BOC, ASCP-BOR, or AMT? (Please submit a copy of your Certification. Failure to submit may result in GS-7 grade consideration only).

A. I possess the appropriate certification as a MT, MLS, or CLS given by the ASCP-BOC, ASCP-BOR, or AMT and have attached a copy of said Certification.

B. I possess the categorical or specialist technologist level certification from the certification agencies listed above, such as, but not limited to, chemistry (C/SC), hematology (H/SH), microbiology (M/SM), molecular biology (MB/SMB) and blood bank (BB/SBB) and have attached a copy of said Certification.

C. I do not currently possess certification as described above but meet all other requirements for GS-7 consideration (Non-certified applicants who otherwise meet the eligibility requirements for certification may be given a temporary appointment up to the full performance level not to exceed one year as a graduate medical technologist ? NOTE: For this announcement, the ?Exception Waiver? will only apply to GS-7 grade level consideration).

D. I do not currently possess certification, but I am a past, current or upcoming VHA Health Professional Trainee Program participant. (Documentation required with application; e.g. SF-50 or VA Scholarship letter.)

E. I have been continuously employed as a Medical Technologist with the Department of Veterans Affairs since May 11, 2014 and qualify via the Grandfathering Provision. I have attached my SF50s as verification.

F. I do not have a certification as described above and do not otherwise meet the eligibility requirements for certification.

4. Please provide information regarding Certification: If you are currently certified, please advise as to the following details regarding the National Certification you possess to include: Type of Certification, State Issued, Date Issued, Expiration Date and Certification Number(if applicable). Note: For GS-9 Consideration, you must upload a copy of applicable Certification. Failure to provide proof of Certification may limit you to GS-7 grade consideration only.

5. GS-9 EXPERIENCE (Full Performance): In addition to meeting the basic requirements above, you must also meet the minimum experience or education for GS-9 consideration. Select the statement below that best describes your ability to meet the grade requirements for this position at the GS-09 level.

A. I have at least 1 year of creditable experience equivalent to the next lower grade level, which is directly related to the position. Examples of experience for this position include serving in an entry level MT career development position practicing under close supervision of an MT at or above the full performance level. B. I have a master’s degree or 2 academic years of progressively higher-level graduate education in medical technology or directly related field. (Note: If you are using this as a 'Qualifying Factor', Proof of education [unofficial transcript] should be submitted with your application).

C. I do not meet any of the above, therefore I may only be eligible at the GS-07 level.
