Accomplishment Record Instructions:  You are required to submit an Accomplishment Record that meets the criteria of the 5 following steps:

It is important to take sufficient time to complete each of these steps thoroughly and accurately.  Omission of any information may result in a significantly lower rating and make it impossible to evaluate your accomplishment and compare it to the accomplishments of other applicants.

            -What conclusions were reached and how were they supported by evidence?
            -What audience did you communicate the results to and how did you communicate with that audience?

B.  Describe a policy or position paper you have either written or had a major role in writing.  Your description should include:

            -The agency or other organization for which the document was written
            -The reason that a policy or position paper was needed
            -How you planned and structured the process of writing the document
            -How existing policy or other information influenced your work
            -What kind of new information was gathered and how its accuracy was determined
            -What new guidance or requirements were included and why were they appropriate?
            -What audience did you communicate the results to and how did you communicate with that audience?

Step 4:  Provide the name and contact information (phone and/or email) of at least one person who can verify your description.

This contact does not need to be your supervisor, but should be someone who worked closely with you and can verify your description of the accomplishment and your role in it.  Often a colleague or client can provide this information as well as a supervisor. Remember that your accomplishment record cannot be rated if it cannot be verified by someone who has first-hand knowledge of the accomplishment and your role in achieving it.

Step 5:  Upload a document that includes the accomplishment record, a link to, or a point of contact to obtain the final product you produced for this accomplishment. As you move through the application process there will be a document upload page where you can upload this Accomplishment Record document.
