It is important to take sufficient time to complete each of these steps thoroughly and accurately. Omission of any information may result in a significantly lower rating and make it impossible to evaluate your accomplishment and compare it to the accomplishments of other applicants.

Step 4:  Provide the name and contact information (phone and/or email) of at least one person who can verify your description.

This contact does not need to be your supervisor, but should be someone who worked closely with you and can verify your description of the accomplishment and your role in it. Often a colleague or client can provide this information as well as a supervisor. Remember that your accomplishment record cannot be rated if it cannot be verified by someone who has first-hand knowledge of the accomplishment and your role in achieving it.

Step 5:  Upload a document that includes the accomplishment record, a link to, or a point of contact to obtain the final product you produced for this accomplishment.
