This position requires the completion of a pre-employment Physical Examination and an annual examination thereafter to ensure the continued, required level of physical health & ability or fitness to perform the duties of the position. This agreement serves as a contract between the Pathways Participant and their supervisor and includes details around roles and responsibilities, work schedules, and training and program requirements. If selected, will you sign a Pathways Participant Agreement and adhere to all program requirements?

To be eligible for the Pathways Student Internship Program, you must be in good academic standing, possessing a Grade Point Average (GPA) of 2.0 or higher. Are you currently a student in good academic standing and understand you must remain in good academic standing throughout your Internship?
