  1. was married to the service member on the date the service member died, while on active duty in the Armed Forces; AND

(2) DD-214 documenting the death of the service member while on active duty, the death certificate; or a letter from the Department of Veterans Affairs or Armed Service, etc. specifying the service member's death while on active duty.
