I have the ability to demonstrate dependability such as reporting daily to the workplace on time and on schedule, self-motivation, positive work ethic and pride of workmanship and receive on-the-job training to learn assigned tasks. I work on my own initiative under general supervision subject to occasional inspection.

C. I have the ability to demonstrate reliability by reporting to work or school on a regular and recurring schedule without incidents such as poor performance, tardiness, attendance or unprofessional behavior.

D. I have the ability to report to work or school but with prior incidents such as poor performance, tardiness, attendance or unprofessional behavior.

E. I do not possess the abilities described above.
