1. EDUCATION REQUIREMENT: Select the one statement that best describes the education level you possess that demonstrates your ability to perform the work of an Inpatient Pharmacist.

2. LICENSURE: For grades above the GS-11, you must hold a full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia.
  • Exceptions
    • : Non-licensed pharmacists who otherwise meet the eligibility requirements may be given a temporary appointment at the entry level as a Graduate Pharmacist under the authority of 38 U.S.C. 7405(c)(2)(B).
    • Grandfathering Provision: All licensed pharmacists employed in VHA in this occupation on the effective date of this qualification standard are considered to have met all qualification requirements for the title, series and grade held, including positive education and licensure/certification/registration that are part of the basic requirements of the occupation.
      • Note: Applicants at the GS-12 grade level must possess licensure.

4. EXPERIENCE, EDUCATION, AND LICENSURE: GS-11 Pharmacist.

There are no requirements beyond the basic requirements i.e., English language proficiency, Citizenship, Licensure and Education requirement.
  • NOTE: Exception. Non-licensed pharmacists who otherwise meet the eligibility requirements may be given a temporary appointment at the entry level as a Graduate Pharmacist. The appointing official may waive the requirement of licensure for a period not to exceed 2 years for a pharmacist that provides care under the supervision of a licensed pharmacist. For grade levels above the GS-11, the candidate must be licensed.

5. EXPERIENCE REQUIREMENT GS-12 Clinical Pharmacist:

In addition to the basic requirements, you must also possess one of the requirements below:
  1. Possess one year of experience equivalent to the next lower grade level (GS-11). Examples of qualifying experience includes, but not limited to: Identifying and resolving customer's concerns accurately and in a timely manner, Reviewing prescriptions to assure accuracy, to ascertain the needed ingredients, and to evaluate their suitability, Updating or troubleshooting pharmacy information databases, analyzing prescribing trends to monitor patient compliance and to prevent excessive usage or harmful interactions, educating patients on the purpose, proper use, and expected outcomes of prescribed medications, protecting information from unauthorized release, loss, alteration or deletion, and following applicable regulations. OR
  2. Successful completion of an Association for Accreditation Council for Pharmacy Education (ACPE) accredited Pharm.D. program.
