An HPT may be VA-paid, Without Compensation (WOC), or a participant of the VA Health Professional Scholarship Program that has been appointed to a temporary position in one or more VHA medical facilities performing clinical or research training experience to satisfy program or degree requirements.
To verify your standing as a current/past/upcoming HPT graduate, you must submit a copy of your latest Notification of Personnel Action, Standard Form 50 (SF-50), the FL-294 (or a WOC appointment letter).

Thank you for your interest in the Clinical Pharmacist position at the CW Bill Young VA Medical Center in Bay Pines, Florida. Your resume and the responses you provide to this assessment questionnaire will be used to determine if you qualify for this position. BASIC REQUIREMENT: Answer the following questions by selecting the appropriate response. These questions will determine if you meet the basic requirements required for appointment as a Pharmacist with the Department of Veterans Affairs.

EDUCATION REQUIREMENT: Select the one statement that best describes the education level you possess that demonstrates your ability to perform the work of an Inpatient Pharmacist.

LICENSURE: For grades above the GS-11, you must hold a full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia.

- Exceptions
- : Non-licensed pharmacists who otherwise meet the eligibility requirements may be given a temporary appointment at the entry level as a Graduate Pharmacist under the authority of 38 U.S.C. 7405(c)(2)(B).
- Grandfathering Provision: All licensed pharmacists employed in VHA in this occupation on the effective date of this qualification standard are considered to have met all qualification requirements for the title, series and grade held, including positive education and licensure/certification/registration that are part of the basic requirements of the occupation.

- Possess one year of experience equivalent to the next lower grade level (GS-11). Examples of qualifying experience includes, but not limited to: Identifying and resolving customer's concerns accurately and in a timely manner, Reviewing prescriptions to assure accuracy, to ascertain the needed ingredients, and to evaluate their suitability, Updating or troubleshooting pharmacy information databases, analyzing prescribing trends to monitor patient compliance and to prevent excessive usage or harmful interactions, educating patients on the purpose, proper use, and expected outcomes of prescribed medications, protecting information from unauthorized release, loss, alteration or deletion, and following applicable regulations. OR
- Successful completion of an Association for Accreditation Council for Pharmacy Education (ACPE) accredited Pharm.D. program.
