Thank you for your interest in an Explosives Worker (Forklift Operator) position with Department of the Army. Your resume and the responses you provide to this assessment questionnaire will be used to determine if you are among the best qualified for this position. Your responses are subject to verification. 1. Please select the response below that best indicates how you meet the screen-out element for the Explosives Worker (Fork Lift Operator), WG-6502-05. Failure to meet this screen-out element will result in an ineligible rating.

A. I have independently operated an automatic machine to perform demilitarization of conventional ammunition such as a vacuum machine to draw off powder charges or a pull-apart machine to break down fixed cartridge ammunition. I have disassembled ammunition components, fuses, base plugs and removed safety or arming wires or strings. I have operated electric, gasoline, or diesel powered fork lift trucks which are capable of lifting loads weighing up to 10,000 pounds.

B. With minimal supervision, I have performed manual operations involving the disassembly or reassembly of ammunition. I have used wrenches and other common tools to tighten end plugs, assemble a fuse, inserting booster charges and detonators, screwing or bolting down components to ammunition equipment. I have operated electric, gasoline, or diesel powered fork lift trucks which are capable of lifting loads weighing less than 10,000 pounds.

C. Without more than normal supervision, I am able to perform the common duties/tasks associated with the position of an Explosives Worker (Forklift Operator). I have experience following safe and proper methods of handling ammunition or explosives or movement/storage/verification of hazardous waste. I have operated an electric, gasoline, or diesel powered fork lift truck.

D. I have the ability to perform basic or simple tasks associated with the work of an Explosives Worker (Fork Lift Operator): performing basic material handling duties. However, I know little or nothing about the duties identified in “A”, “B”, or “C” above.

E. My training or experience is not reflected in the above statements.
