6. Outpatient Mental Health Division/Behavioral Health Interdisciplinary Program (BHIP): Specific responsibilities include but are not limited to direct patient care, medication assisted treatment of substance use disorder, suicide risk screening and observation of high risk Veterans, screening and evaluation of behavioral consults, triage of secure messaging, phone calls for Veterans receiving outpatient mental health services and assessment and triage of Veterans who seek walk-in/urgent mental health care. 7. Domiciliary (DOM)/Mental Health Residential Rehabilitation Program (MHRRTP): Specific responsibilities include but are not limited to direct patient care, assessment, planning and evaluations, use of medical equipment, medication management and recognition of mental health or medical emergencies for Veterans admitted to one of three residential units. The Domiciliary is a SUD (substance use disorder) program which also houses veterans with mental illness, homelessness, and substance abuse disorders. A minimum of one year of mental health experience is preferred but not mandatory. (limit characters)
8. Community Living Center Skilled Nursing and Rehabilitation Unit: Provides compassionate care to Veterans from young adult to geriatric years with sufficient functional impairment that require a level of service and skill available in the CLC/SR. Veterans with chronic stable conditions requiring rehabilitation or short-term specialized services, dementia/memory impairment, and hospice/palliative care are served in the CLC/SR. The nurse provides direct and indirect care to patients receiving these services, helping the Veteran return to an independent level of functioning. Hospice: Providing end of life care to Veterans admitted to inpatient hospice unit on CLC. The nurse provides direct and indirect care to Veterans, working with an interdisciplinary team to maintain comfort, both emotional and physical to the Veteran and family members.

9. Home Based Primary Care (HBPC): Home visits required, and part of interdisciplinary team focused on providing care to eligible Veterans in their home.

10. Home Telehealth: Support veterans through telemonitoring to Veteran's home.

11. Caregiver Support Program: RN home visits required to monitor and support Veterans in the Caregiver Support program.

12. Community Nursing Home: RN required to make visits to Contract Community Nursing Home for oversight visits.

13. Non-Institutional Care (NIC) Program: Support Veterans by doing care coordination with Veterans and Community Agencies for home care, hospice, and respite.

15. Homeless Patient Aligned Care Team (PACT): Specific responsibilities include but are not limited to providing appropriate, comprehensive and cost-effective primary care services to homeless Veterans in alignment with the National VHA goal to prevent and end homelessness. Addressing competing factors and needs of homelessness, enhancing accessibility and facilitating Veteran engagement in healthcare services, building longitudinal relationships and aligning clinical treatment plans with housing agenda.

18. Med/Surg (3B): A Medical Surgical floor focusing on pre- and post-operative surgical patients with specialties to include Urology, Orthopedics, Podiatry, GI, Women's Health Services and General Surgery. 3A also supports the medicine service for cases of general medicine, internal medicine, psychiatry, co-morbidities, end of life, palliative care, hospice, wound care and cardiac care.

19. Emergency Department (ED): ED Nurse Recruitment statement: The ED nurse will provide emergent/urgent care for Veterans presenting to the ED. Patients ranging in age from young adulthood to geriatric years who arrive to the hospital in need of immediate care to include: rapid triage, stabilization of emergent situations, care of the mentally ill patient, and any other acute need for the diverse Veteran population seeking help. Prefer minimum of 2 years Emergency Department nurse experience but not mandatory.

20. Medicine Sub-Specialty Clinics: Typically assigned to but not limited to Cardiology, Gastroenterology, Urology, Hematology/Oncology, Infusion, Podiatry and Surgery clinics. Each specialty team will include the physicians, nurse practitioners, RN(s), and Health Techs. Primary duties include managing the patient care environment of the clinic, supplies, and patient flow.

21. Specialties & Diagnostics: Pre & Post-Operative Care provides care for Veterans ranging in age from young adulthood to geriatric years. As part of the Peri-Operative experience, the surgical nurses' complete pre-operative assessments and prepare the Veteran for their surgical procedures as well as the post-operative assessment and recovery of each Veteran from sedation, if administered. Procedure Room - duties include nursing assessment and direct care for Veterans ranging in age from young adult hood to geriatric years who require procedural interventions for multiple specialties. Provides moderate sedation in specific specialty areas. Gastroenterology or Endoscopy provides care for Veterans ranging in age from young adulthood to geriatric years. As part of the Peri-Operative experience, the surgical nurses' complete pre-operative assessments and prepare the Veteran for their surgical procedures as well as the post-operative assessment and recovery of each Veteran from sedation. Provides moderate sedation intra-operatively during Endoscopy procedures.

22. Float Pool: The Float Pool Program encompasses a team of Registered Nurses (RN) working full-time (12 hour shifts day or night) to substitute for absent full-time staff in various departments (3B/4B Acute Care, ED, Community Living Center (CLC), and the Domiciliary) within the Northern Arizona VA Health Care System, Prescott Campus. The RN reports to the Coordinating & Support Staff Nurse Manager, but must work in collaboration with the Assistant Nurse Managers, Charge Nurses, Staff Nurses, Health Techs, Medical Support Assistants, and providers of the assigned team.
