Submit a copy of your most recent completed ANNUAL performance appraisal, which includes 1) all associated pages, 2) the final rating, and 3) supervisory signature. The performance appraisal must be signed and dated within the past 18 months. If you have changed positions within the fiscal year, you may need to provide an additional appraisal to capture your final rating and the details of your performance in each element. This must document a full fiscal year of rating to be accepted.
If you do not have a performance appraisal that meets all of the requirements above or if you have not received an annual performance appraisal for any reason (not employed, current employer does not give a performance appraisal, etc.), you MUST explain why when asked in the vacancy questionnaire. N/A is not a suitable answer. A performance plan may not meet the definition of your final rating if all requirements are not on the document.
