I have a Doctor of Dental Medicine (DMD) or Doctor of Dental Surgery (DDS) degree from a school approved by the American Dental Association (ADA) or the Commission on Dental Accreditation (CODA) or an accrediting body recognized by the U.S Department of Education at the time the degree was obtained. (Note: You must attach a copy of your transcripts.)

B. I do not possess the education described above.

The Dentist occupation requires passing of the National Board Dental Examinations (NBDE) which are developed and administered by the Joint Commission on National Dental Examinations (JCNDE) and possess a current, full and unrestricted license to practice dentistry in a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. Select the one statement that best describes the education and/or qualifying experience that you possess that demonstrates your ability to perform the work of the Supervisory Dentist at the GP-14 grade level or equivalent pay band in the Federal Service. (Qualifying experience involves the evaluation, prevention, diagnosis, and treatment of diseases, injuries, and disorders of the oral cavity and maxillofacial area. Examples of dentistry experience may include the removal of decay; filling cavities; straightening, extracting, and repairing teeth; treating gum diseases; performing corrective surgery; making measurements/models for dentures to replace missing teeth, administering anesthetics, writing prescriptions; and performing other requirements of the position.

A. I have completed a 3-year residency in dentistry, oral health, craniofacial sciences, or a related medical science, AND one year of full-time experience and/or training in professional dentistry work equivalent to the next level grade level (GP-13).

B. I have four years of professional dentistry experience and/or training, equivalent to the next lower grade level (GP-13).

C. I have completed a residency program in dentistry, oral health, craniofacial sciences, or a related medical science AND professional dentistry experience and/or training equivalent to the next lower grade level (GP-13). The combination of my residency program and professional dentistry experience and/or training is equal to or greater than four years.

D. I have the education and/or experience as described in A and in B.

E. My education and/or experience is not reflected in any of the above statements.
