Appointment to this position is subject to a background investigation to determine your suitability for Federal employment. Are you willing to submit the proper documentation to have a background investigation completed?

This position requires the completion of a pre-employment Physical Examination and an annual examination thereafter to ensure the continued, required level of physical health & ability or fitness to perform the duties of the position. The duties of this position meet the criteria for compliance with Federal and Department of the Army Drug Free Workplace Programs. Detection of the presence of drugs is warranted. This position is subject to a pre-employment screening, and random testing thereafter, to include testing based on reasonable suspicion and testing due to direct involvement with an on-duty accident. This position has been designated "Mission Essential". In the event of severe weather conditions or other such emergency type situations (natural or man-made disaster) the incumbent is required to report to work or remain at work as scheduled to support mission operations. The duties of this position require the incumbent to possess or obtain and maintain a valid state Driver's License in one of the 50 U.S. states or possessions to operate vehicles. This position has a requirement to lift up to 50 lbs. in the regular performance of duties. Are you willing and able to meet the lifting requirement?

This position requires shift work to provide coverage on evenings, weekends, holidays, or special situations. Employee shifts are rotated on a regularly scheduled basis. Are you willing and able to work on a rotating shift basis to meet work requirements?

This position requires the incumbent to work 72 hours per week (144 hours per pay period) including 24-Hour rotation shifts, working nights, weekends and holidays. I understand this requirement and am willing to work the schedule as stated above.

This position has been designated "Inclement". Incumbent is required to report for duty during inclement weather, Natural Disasters, and other emergency situations as directed by the Fire Chief/Commander and may be required to remain on duty after the official shift ends, i.e., mandatory overtime; subject to recall to duty. Incumbent must maintain a functioning telephone at their residence to aid in contact during recall/emergency situations. Within 180 days of occupying this position, you must obtain Wildland Firefighter Type II (NWCG Red Card) certification. I confirm that I will obtain this certification within 180 days of hire. Are you willing to meet this requirement?

Employees who have completed IFSAC or Pro-Board accredited training programs may request DOD equivalent certification certificates under the reciprocity provision of the certification program. The certificates being submitted for Change 1, 09/16/2010 25 CHAPTER 4 DOD 6055.06-M, February 23, 2006 reciprocity must have the appropriate IFSAC or Pro-Board seal on them. The DOD Fire and Emergency Services Certification Program shall not issue certification certificates for completing non-accredited training.

At the GS-07 level, this position requires a valid Emergency Medical Technician (EMT) - Basic license (Must meet Washington State EMT or National Registry requirements).
